About This Questionnaire

This questionnaire is designed to help you reflect on your sexual interests, desires, and curiosities in a safe, private manner. Your responses are confidential and will only be used to guide your therapeutic journey if you choose to share them.

There are no "right" or "wrong" answers, and this is not a diagnostic tool. It's simply a way to explore your thoughts and feelings about sexuality and intimacy.

You can complete this form in private and decide later whether you'd like to discuss any of these topics with your therapist.

• Take your time answering each question thoughtfully.
• For each statement, indicate your level of interest, curiosity, or comfort.
• If a question makes you uncomfortable, you may skip it.
• Your responses are completely private unless you choose to share them.
• This form can be saved and completed at your own pace.

This questionnaire is for adults (18+) only and is intended for personal reflection or discussion with a qualified therapist.

Sexual Curiosity Questionnaire

Please answer the following questions as thoroughly and honestly as you feel comfortable with. You can take your time and return to complete this form later if needed.

1. What qualities would you like to have more of in your sexual relating (both in yourself and in your partner(s)?

2. What qualities would you like to develop and explore in yourself as a sexual and relational being?

3. What, if anything, would you like to have more of?

4. If you could describe your perfect sexual relationship, what would it look like?
